When it comes to improving advanced signs of aging and skin damage, few treatments are as transformative as deep CO₂ laser resurfacing. This intensive treatment addresses concerns that lighter lasers and superficial treatments may not fully correct, including deeper wrinkles, significant sun damage, uneven texture, and acne scarring.
Deep CO₂ resurfacing requires more downtime, but for the right patient, the improvement can be dramatic and long-lasting.
What Is Deep CO₂ Laser Resurfacing?
A CO₂ laser uses a wavelength of light that is absorbed by water in the skin. Because skin contains water, the laser can precisely remove damaged surface tissue while delivering controlled heat into the deeper layers.
This creates two important effects:
- Damaged outer layers of skin are removed.
- The skin’s natural healing response is activated, stimulating new collagen and elastin.

As the skin heals, it becomes smoother, firmer, and more even in tone and texture. Collagen remodeling continues for several months after treatment, so results gradually improve well beyond the initial healing period.
What Can Deep CO₂ Improve?
Deep CO₂ resurfacing may improve:
- Moderate to deeper facial lines and wrinkles
- Crepey skin
- Sun damage
- Rough or uneven texture
- Acne scars and certain other scars
- Enlarged-looking pores
- Uneven pigmentation
- Mild skin laxity
- Overall skin quality
It is especially effective around the mouth and under eyes, where etched-in lines can be difficult to treat with injectables or lighter laser treatments alone.
Although the skin may appear firmer after treatment, CO₂ resurfacing is not a replacement for a facelift. It improves the quality of the skin but cannot remove a significant amount of loose or sagging tissue.

Post-Treatment Expectations
After treatment, the skin will feel hot and appear intensely red. Swelling is expected and may be most noticeable around the eyes. The skin may also ooze slightly during the earliest stage of healing. These responses are normal following an aggressive ablative treatment. Patients are required to heal their skin in ointment 24 hours/day for at least 7 days while they are healing.
How Much Downtime Should I Expect?
Deep CO₂ resurfacing requires true social downtime. Most patients should plan for approximately 7 to 10 days of initial healing.
During the first several days, the skin may be:
- Red
- Swollen
- Tender
- Itchy
- Peeling or flaking
- Crusted in certain areas
New skin commonly covers the treated area within approximately 7 to 10 days, but redness or pinkness may persist for several weeks or longer. Makeup can usually be resumed only after the skin has fully closed and the treating provider has given approval. Full recovery takes longer than the initial peeling stage. It is important to schedule deep CO₂ treatment when you can commit to the complete healing process—not immediately before a wedding, vacation, or major event at work.
When Will Patients See Results?
The first improvement becomes noticeable once the skin has healed and the initial redness begins to settle. The complexion typically appears smoother, brighter, and more refreshed.
Results continue to develop as new collagen forms and remodels, which may take several months. Improvements can be long-lasting, although the skin will continue to age naturally.
Daily sunscreen, medical-grade skincare, and healthy lifestyle habits will help protect and maintain your results.
Who Is a Good Candidate?
Deep CO₂ resurfacing is appropriate for someone who:
- Has moderate to advanced skin damage
- Wants significant improvement in texture or wrinkles
- Understands and accepts the required downtime
- Can closely follow pre- and-post-treatment instructions
- Is willing to practice strict sun protection
- Has realistic expectations
A detailed consultation is essential. Your provider will review your medical history, medications, previous treatments, tendency to develop cold sores, history of abnormal scarring, and any past pigmentation problems.
Patients with darker skin tones or melasma may have a greater risk of post-inflammatory pigmentation. This does not mean every patient is automatically excluded, but treatment selection and preparation require additional caution and expertise. In some cases, a different procedure may be safer.
Is Deep CO₂ Right for You?
Deep CO₂ resurfacing is not a casual “lunchtime” treatment—it is an investment in substantial skin renewal. For appropriately selected patients who are prepared for the recovery process, it can significantly improve wrinkles, texture, scarring, sun damage, and overall skin quality.
The best treatment is not necessarily the most aggressive one. It is the treatment that safely matches your skin, concerns, lifestyle, and goals.
